Enhancing Care for Older Adults Through Delirium Prevention Initiatives

Campbellford Memorial Hospital (CMH) is strengthening its approach to caring for older adults through a series of focused initiatives aimed at preventing delirium and supporting patients who experience it during their hospital stay.

Delirium is a serious and often preventable condition that can cause sudden confusion and changes in awareness, particularly among seniors. At CMH, a proactive, team-based approach is helping reduce risk, support early recognition, and improve the overall care experience for patients and their families.

Education on delirium prevention is provided to staff across the organization and shared with patients and caregivers at the time of admission, ensuring everyone understands how to recognize early signs and contribute to care.

In the Emergency Department, practical tools have been introduced to help patients remain oriented and comfortable, including a Senior Care Comfort Cart, large patient-facing clocks, and easy-to-read bedside calendars. On the Inpatient Unit, whiteboards are used to reinforce key information, while early mobilization and therapeutic activities, led by the Restorative Care Team, help patients stay active and engaged.

Additional support is provided through a dedicated Behavioural Supports Ontario (BSO) Registered Nurse, ensuring patients experiencing delirium receive timely, specialized care tailored to their needs.

“Our focus is on creating an environment that supports orientation, mobility, and meaningful engagement, three key factors in preventing and managing delirium,” said Cathy Galbraith, CMH’s Manager of Seniors Health and Restorative Care. “These are often simple interventions, but they have a significant impact on patient safety, recovery, and overall well-being.”

“Delirium doesn’t just affect the patient, it impacts families as well,” Galbraith added. “By equipping staff, patients, and caregivers with the right knowledge and tools, we’re able to respond earlier and provide more compassionate, effective care.”

Through these initiatives, CMH continues to build a more senior-friendly environment, improving outcomes and enhancing the hospital experience for older adults and their families.
