Campbellford Memorial Hospital is committed to protecting your privacy.

In order to provide you with the best possible care, it is important that we have access to your personal health information.

We collect, use, disclose, retain and protect your information in keeping with the Personal Health Information Protection Act (2004).

You have the right to know how we may use and disclose your personal health information and how you can access it. Hospital staff are not permitted to collect personal health information unless it is necessary, or to collect, use or disclose more information than required.

You may request access to and/or correction of your personal health records, or withdraw consent for certain uses.

As part of our Privacy Program, we provide ongoing privacy education for staff, patients and visitors, and have a designated Privacy Officer at CMH. See the link below for our Privacy Policy.

Requesting Access to Information under FIPPA

You may request access to information at Campbellford Memorial Hospital under the Freedom of Information and Protection of Privacy Act (FIPPA). FIPPA provides members of the public with a general right of access to information within the hospital’s custody or control as of January 1, 2007 (with limited exceptions and exclusions).

Anyone can make a formal request under FIPPA by mailing or delivering a written request to Campbellford Memorial Hospital Health Records at:

For your request to be considered complete it must meet the following criteria:

  • Be made in writing and directed to Campbellford Memorial Hospital Health Records.
  • Include a $5.00 application fee (cash, cheque or money order payable to Campbellford Memorial Hospital).
  • Provide sufficient detail to allow staff to locate the requested information with a reasonable search. You may be contacted if additional information is required.

In addition to the $5 application fee, the fees listed below may also apply. If processing costs are expected to exceed $100, you will receive a fee estimate and be required to pay a 50% deposit before processing begins.

Search Time $7.50 per 15 minutes required to search and retrieve records.
Record Preparation $7.50 per 15 minutes required to prepare records for release.
Photocopying $0.20 per page
Computer Programming $15.00 per 15 minutes if required to develop a program to retrieve information.
Discs / CDs $10.00 each
External Costs Any additional costs incurred in responding to a request.
Shipping Actual delivery costs as requested.
