Partnerships Help Campbellford Memorial Hospital Keep Services Closer to Home

CMH’s vision is to be a recognized leader in rural health care, creating a healthy community through service excellence, effective partnerships and the development of innovative hospital partnerships.

Our rural community continues to grow and residents from the Municipality of Trent Hills only represent about half of the CMH’s patient load. The hospital serves a much larger catchment area including three separate counties with approximately 80% of patients coming from a 40km radius of Campbellford. One-third of CMH’s day surgery patients come from Quinte West and Brighton. These statistics underline the central role that the CMH plays in providing health care services in the Region – beyond the borders of Trent Hills.

As a Rural Health Hub serving this largely rural community, CMH is actively initiating and creating collaborative partnerships with community health care providers as follows:

  • Emergency, Inpatient Care and Outpatient Services provided by CMH (i.e. acute care, restorative care, day surgery, diagnostic imaging and lab testing)
  • Comprehensive Primary Care provided by Trent Hills Family Health Team with a strong focus on population health and chronic disease management
  • Home and Community Long-Term Care as well as End of Life Care provided by Campbellford Memorial Multicare Lodge and area agencies such as the Central East Community Care Access Centre and Community Care Northumberland
  • Mental Health and Addictions provided by Campbellford and District Community Mental Health Centre located in the hospital and delivering community-based treatment and support services. This includes: Early Psychosis Intervention for 1,332 clients; Crisis Intervention for 152 clients; Case Management for 439 clients; Counselling & Treatment for 2,596 clients; and Court Diversion and Support for 189 Clients from April 1, 2014 to March 30, 2015.

Together, this group of partners ensures that comprehensive, coordinated patient and family-centred care is provided to meet local needs and our patients/clients receive the right care in the right place at the right time. This approach is leading to quality care improvements as noted in the following examples and success stories:

Improving Access to Services Closer to Home:

  • CMH is partnering with Community Living Campbellford/Brighton (CLC/B) to address the current service gap for adults with intellectual disabilities. This is a growing demographic that requires higher levels of care. The project will review CLC/B’s existing practices which may perpetuate an unintentional systemic challenge for aging people. The project will also foster the pursuit of CLC/B partnerships with CCAC, Central East LHIN, Canadian Mental Health Association and Ontario Partnership on Aging & Developmental Disabilities to further close the existing services gap.
  • CMH led the creation of a Trent Hills palliative collaboration committee, including Community Care Access Centre, Trent Hills Family Heath Team, Community Care Northumberland, and Bridge Hospice, to provide a “wrap around” palliative care service. The outcome is a common referral process, team-based education, and removal of barriers between stakeholders. CMH also created a direct admission process for palliative patients.
  • Our new Geriatric Assessment Intervention Network (GAIN) Community Team is ensuring we’re meeting the needs of older adults in our community and collaborating with regional partners to explore opportunities for optimum patient outcomes.
