The Trent Hills Family Health Team

The Trent Hills Family HealthTeam is an interdisciplinary primary health care model staffed by 10 physicians, 4 Nurse Practitioners,2Mental Health DepartmentWorkers, shared Dietician services, Registered Nurses, Registered Practical Nurses, Executive Director and support staff.

Three of the 10 physicians formed the Northumberland Family Health Network (NFHN) in 2002. This FHN, one of the first in Ontario, became the core of the Trent Hills Family Health Team (THFHT) approved by the Ministry of Health and Long-Term Care in 2005. Incorporated in 2006, the THFHT is governed by a Board of Directors and has an annual operating budget of approximately $1.7 million.

The main site islocated on the upper floor of the Campbellford Memorial Health Centre with 4 additional sites in Warkworth, Hastings, Colborne and Havelock.The Teamcurrently delivers primary care servicestoapproximately 16,000 patients in Trent Hills and adjacent areas, with a totalTeam of over 60 health professionals and staffover the 5 sites.

The THFHT is continually developing and implementing various programs, services and education for patients. All programs and services are delivered in an interdisciplinary environment using a collaborative care approach. Current Programs include: Diabetes Management (in collaboratin with CMH), Chronic Non-Cancer Pain Management, Anticoagulation therapy monitoring, Palliative Care Coordination, Appointment Wait Time Reduction, Cervical Screening, Flu Immunization, Smoking Cessation (in collaboration with the Ottawa Heart Institute). Current Services include: Phlebotomy (5 days/week), Cognitive Screening, Spriometry, Mammography and Colon Cancer Screening. Social Workers provide Group Sessions for childhood/teen anxiety. Annual patient visits total approximately 74,000.

In collaboration with CMH, Health Professionals have been trained to run the PATH (Palliative and Therapeutic Harmonization) Program and are involved with CMH's new GAIN (Geriatric Assessment and Intervention Network) clinic.

The THFHT has a close partnership with the Campbellford Memorial Hospital and The Bridge Hospice, Warkworth.

Currently, a Nutrition Pilot Program is underway with great results.
