Campbellford Memorial Hospital (CMH) is thrilled to announce an exciting opportunity for community members to join the Patient and Family Advisory Council (PFAC). PFAC is a vital component of our commitment to enhancing the patient and family experience within our healthcare system. By becoming a member of PFAC, you can actively participate in influencing positive change at CMH that makes a meaningful difference to our patients and their families.
Purpose of PFAC:
PFAC is dedicated to amplifying the patient and family voice at CMH. The council's primary focus is to collaborate with key decision-makers to enhance the patient and family experience. PFAC members actively participate in various hospital initiatives, committees, and projects, helping to shape the future of healthcare at CMH.
Scope of Responsibilities:
As a member of PFAC, you will have the opportunity to:
- Participate in Monthly Council Meetings: Engage in discussions and decision-making processes that directly impact patient care and experience.
- Represent PFAC: Serve as a voice for patients and families on hospital and Board of Director committees, ensuring their perspectives are considered in policy and decision-making.
- Identify and Lead Change Initiatives: Collaborate with CMH leaders to identify areas for improvement and actively lead change initiatives that enhance the patient and family experience.
- Consult on Major Decisions: Provide valuable input and feedback on significant hospital decisions and initiatives.
Eligibility and Qualifications:
We are seeking individuals who are passionate about patient-centered care and who have been, or had a family member be, a patient at CMH. While a background in healthcare is welcome, it is not a prerequisite. We value diverse perspectives and encourage individuals from all walks of life to apply.
