The Campbellford Memorial Hospital Diabetes Education Program offers education classes and counseling to help individuals with diabetes achieve better knowledge and awareness of diabetes and its implications. Our program is suitable for adults, aged 18 or over with:

  • Prediabetes
  • Newly diagnosed diabetes
  • Non-insulin dependent diabetes
  • Insulin-dependent diabetes

Our program offers access to an interdisciplinary team which includes a Registered Nurse and Registered Dietitian. The clinic is open from Monday - Thursday, 8:00 am - 4:00 pm. We are closed on all statutory holidays.

We offer an education class each month for patients in the program taught by both the nurse and the dietitian. We encourage all patients in the program to attend this class at least once.

In addition to education classes we also offer counseling with our nurse and dietitian. Most often, your first appointment at the clinic will be with both the nurse and dietitian. After this, individual or group counseling follow-up is scheduled based on your specific needs.

In order to gain access to our program, we accept referrals from all healthcare professionals including: family doctors, specialists, nurse practitioners, pharmacists and dietitians. We are also happy to accept self-referrals. To self-refer please call the diabetes education program at 705-632-2024.
