The Hospital continually evaluates the services provided to our patients. If contacted, please take a minute to provide feedback so that we can continue to provide optimum care.
Emergency Department Survey
In order to assist us with our evaluations, we ask you to take a moment before you leave and complete a survey regarding the care you received at the Hospital.
You may receive a survey in the mail from an outside company regarding your in-patient stay or emergency room visit. Your name is not disclosed to the hospital. Your comments and the survey provide valuable feedback to the hospital.
